Closely associated with the work of director John Waters, Smith is credited with designing Divine's make-up on Pink Flamingos (1972). Other films include Hairspray (1988), Cry-Baby (1990), Pecker (1998) and A Dirty Shame (2004). He also designed some more sober costumes for episodes of Homicide: Life on the Street (1993).
